Chronic Disease Management
Our commitment to community health includes Chronic Disease Management Plans tailored by our doctors for patients with chronic conditions such as:
- Asthma
- Cancer
- Cardiovascular disease
- Diabetes
- Musculoskeletal conditions
- Stroke
A chronic medical condition is one that has been present for at least six months or longer.
These chronic disease management plans, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs), offer structured, ongoing care. GPMPs identify your needs, outline GP services, and suggest actions to manage your condition. TCAs coordinate multidisciplinary care among your GP and other providers.
TCAs require your GP to collaborate with at least two other health or care providers who will give ongoing treatment or services to you. Let your GP or nurse know if there are aspects of your care that you do not want to be discussed with other healthcare providers.
